Thousands of patients who started on daily Saxenda are now asking the same question: is it time to move to once-weekly Wegovy, and how should the switch be done safely? Both medicines are GLP-1 receptor agonists made by Novo Nordisk, but they use different molecules (liraglutide vs semaglutide), different dosing rhythms and different titration schedules. Getting the transition right protects tolerability, keeps weight-loss momentum and avoids common mistakes such as overlapping the two drugs.

If you want a head-to-head look at which drug suits which patient, see our earlier guide, Comparing Wegovy and Saxenda: Which Weight Loss Medication Is Right for You. This article focuses on something different: the practical process of switching from Saxenda to Wegovy, covering timing, starting dose, titration and what patients and clinics should expect in the first months.

Why Patients Switch From Saxenda to Wegovy

The most common reasons clinicians consider a Saxenda to Wegovy transition are:

  • Greater average weight loss. In the STEP 8 randomized trial, adults without diabetes lost a mean 15.8% of body weight on semaglutide 2.4 mg weekly versus 6.4% on liraglutide 3.0 mg daily over 68 weeks (Rubino et al., JAMA 2022).

  • Fewer injections. One injection per week instead of seven makes adherence easier for many people.

  • A plateau on Saxenda. The Saxenda label advises stopping treatment if an adult has not lost at least 4% of baseline weight after 16 weeks, which prompts many prescribers to review alternatives.

  • Supply, cost or insurance changes, which differ by country and change frequently.

Switching is not mandatory. US Veterans Affairs conversion guidance notes there is no need to convert a patient who is doing well and losing weight on liraglutide. The decision should always be individual.

Saxenda vs Wegovy Dosing at a Glance

According to the official US prescribing information:

Saxenda (liraglutide) is injected subcutaneously once daily. It starts at 0.6 mg per day for one week and rises by 0.6 mg each week (1.2 mg, 1.8 mg, 2.4 mg) until the 3 mg daily maintenance dose is reached in week 5. Lower doses are for titration only.

Wegovy (semaglutide) injection is given subcutaneously once weekly, on the same day each week. The escalation schedule is:

  • Weeks 1–4: 0.25 mg

  • Weeks 5–8: 0.5 mg

  • Weeks 9–12: 1 mg

  • Weeks 13–16: 1.7 mg

  • Week 17 onward: maintenance of 2.4 mg (recommended) or 1.7 mg

The current US label also allows adults who have tolerated 2.4 mg for at least 4 weeks to increase to a maximum of 7.2 mg weekly when further weight reduction is clinically indicated. Doses and available strengths vary by country, so always follow the local label. For background on how semaglutide works, read Understanding Semaglutide Injection and Its Uses.

Both labels state that the drug should not be combined with any other GLP-1 receptor agonist. That single rule shapes the whole switching process.

How to Switch From Saxenda to Wegovy: Step by Step

Neither the Wegovy nor the Saxenda label contains an official Saxenda-to-Wegovy switching protocol. What follows reflects published clinical guidance and common practice. The prescriber always makes the final call.

Step 1: Review the Reason for the Switch

Clinicians first confirm why the patient wants to change, whether that is efficacy, convenience, cost or side effects. They then recheck contraindications. Wegovy must not be used in patients with a personal or family history of medullary thyroid carcinoma or MEN 2, and people with type 2 diabetes need glucose monitoring during the change.

Step 2: Timing — No Washout Needed

Practical guidance published in Clinical Diabetes (Almandoz, Lingvay and Morales, 2020) suggests that a patient who tolerates a daily GLP-1 agonist at full dose can start the weekly agent the day after the last daily dose. New Zealand's Christchurch Medicines Information Service (2025) likewise states that no washout period is required. Taking both drugs together is not recommended under either label.

There is one exception. If the switch is happening because of gastrointestinal side effects, the same guidance advises stopping Saxenda and waiting until the symptoms resolve before starting Wegovy at its lowest dose.

Step 3: Choose the Starting Dose

Liraglutide 3 mg daily and semaglutide doses are not directly interchangeable, and there is no validated equivalence. Published conversion tables take a conservative approach:

  • Patients on Saxenda 0.6–1.2 mg daily: usually start Wegovy at 0.25 mg weekly.

  • Patients on 1.8–2.4 mg daily: 0.25 mg or 0.5 mg weekly.

  • Patients stable on 3 mg daily: some services (VA, CMIS) suggest 0.5 mg weekly as an off-label starting point.

Starting at 0.25 mg, as the Wegovy label recommends, remains the most cautious and label-aligned option.

Step 4: Escalate Every 4 Weeks

From the starting dose, Wegovy is stepped up every 4 weeks toward 1.7 mg or 2.4 mg. If a dose is poorly tolerated, the label allows the next escalation to be delayed by 4 weeks. On missed doses, the label says that if one dose is missed and the next is more than 2 days away, it should be taken as soon as possible. If two or more consecutive doses are missed, re-escalation from a lower dose may be needed.

Gastrointestinal Side Effects

Nausea, diarrhoea, vomiting and constipation are the most common adverse effects of both drugs. In STEP 8, gastrointestinal events were reported by 84.1% of semaglutide users and 82.7% of liraglutide users, mostly mild to moderate. However, fewer participants discontinued semaglutide (13.5%) than liraglutide (27.6%). Smaller meals, stopping eating when full and staying hydrated help most patients through each new dose step.

Weight-Loss Trajectory

Because the starting Wegovy dose is low, some patients notice a slight return of appetite in the first weeks after stopping 3 mg Saxenda. This usually settles as the dose rises. In STEP 8, 70.9% of participants on semaglutide achieved at least 10% weight loss, compared with 25.6% on liraglutide. Results still depend on diet, activity and individual biology.

Facial and Body Changes

Faster, deeper weight loss can reveal volume loss in the temples, cheeks and jawline, the so-called "Ozempic face". Aesthetic clinics increasingly pair weight-management patients with restorative treatments. Our article Can Dermal Fillers Reverse Ozempic Face? Causes and Treatment Options explains the options.

Common Mistakes When Changing From Saxenda to Wegovy

  • Overlapping the two drugs. Do not take a final Saxenda dose on the same day as the first Wegovy injection.

  • Jumping straight to 2.4 mg. Skipping titration raises the risk of severe GI reactions and dehydration-related kidney injury.

  • Treating doses as equivalent. 3 mg of liraglutide is not "the same as" any particular semaglutide dose.

  • Ignoring diabetes medication. Patients on insulin or sulfonylureas may need dose adjustments to avoid hypoglycaemia.

  • Using unregulated "semaglutide" products. Patients should use only authorised, prescribed products from legitimate pharmacies.

What This Means for Clinics and Distributors

For aesthetic and wellness clinics, the rise of weekly GLP-1 therapy is reshaping patient journeys. Patients on medical weight management often seek body-contouring, skin-quality and volume-restoring treatments during or after their programme. Our overview of Top Non-Surgical Body Contouring and Weight Management Trends for 2026 maps where the demand is moving.

Brands and distributors building a weight-management portfolio should pair prescription-only GLP-1 care, which stays under medical supervision, with compliant adjunct products such as fillers, collagen stimulators, mesotherapy and body-contouring injectables. FAQ

Can I start Wegovy the day after my last Saxenda injection?

In most cases, yes. Published guidance states that no washout is required, and the weekly agent can begin the day after the last daily dose. If you are switching because of side effects, your prescriber may ask you to wait until the symptoms settle.

What Wegovy dose should I start with after Saxenda 3 mg?

The Wegovy label starts everyone at 0.25 mg weekly. Some clinical services consider 0.5 mg off-label for patients stable on Saxenda 3 mg. Your prescriber decides based on your tolerance.

Can I take Saxenda and Wegovy together?

No. Both labels state that combining them with any other GLP-1 receptor agonist is not recommended.

How long does it take to reach the full Wegovy dose?

From 0.25 mg, reaching 2.4 mg takes 16 weeks, with maintenance from week 17. Starting at 0.5 mg shortens this by about 4 weeks.

Is Wegovy more effective than Saxenda?

In the STEP 8 head-to-head trial, semaglutide 2.4 mg produced an average 15.8% weight loss versus 6.4% with liraglutide 3.0 mg at 68 weeks. Individual results vary.
